We currently see individuals with one or more of these chronic conditions:
1. High Blood Pressure
2. High Cholesterol
3. Pre-diabetes
4. Diabetes
5. Gout
6. Osteoarthritis
7. Allergic Rhinitis
(We will progressively add on to the list of chronic conditions in the near future.)

Our health partners - Kingsway Medical Clinics will be able to provide CHAS subsidies under Chronic Disease Management Programme (CDMP). Do note that there are some requirements specified by MOH in order to utilize the subsidies:

1. First Consultation: The first consultation will have to be a physical in-clinic consult with our health partners at Kingsway Medical Clinics. (Note: The Consultation Fee can be subsidised under CHAS).
2. Subsequent Consultation(s): A maximum of (3) tele-consults may be claimed under CDMP in a calendar year in which you may utilize our app for refills. During this period, our doctors will continue to assess your eligibility for refills over telemedicine, and follow up with you periodically.  
3. Yearly blood test: To monitor for control of your chronic condition, as well as to screen for any complications. Our package costs a nominal fee of $38 - comprising Blood Pressure, Cholesterol, Diabetes, Kidney Function, Liver Function and Urine Protein (Albumin) test with a Doctor's consultation ($0). When you arrange for your yearly blood test with our Doctors, this can double up as your First Consultation with us.

In general, if you meet these criteria (Three C's), our doctors may assess you to be suitable for a refill without a repeat consult. In which case, we will deliver your medications straight to you.

1. Control: Your chronic condition must be shown to be well-controlled with stable trends on your Health Diaries.
2. Compliance: As part of managing your chronic condition safely via tele-consults, you will need to be compliant with your medications (i.e. taking medications regularly). 
3. Complications: If you are experiencing symptoms or complications as a result of your medications or chronic condition, our doctors may arrange a video consultation with you and/or may advise an in-person consultation where necessary and appropriate.

For medication orders processed before 3pm, we will provide next working day delivery. We work alongside our licensed Pharmacy Partners (WellAway Pharmacy) will process the received e-prescriptions and deliver the medications directly to you:

1. Next-working day delivery slots are as follows:
   – 10 am to 2 pm
   – 2 pm to 6 pm
   – 6 pm to 10 pm
   Our customer service team will reach out to you to confirm your delivery slot.
2. Once you have received the medicines, if required, a qualified pharmacist from Wellaway Pharmacy may provide you advice remotely on drug interactions and medicine management.
3. We do not deliver to these secured zones: Airline Road, Airport Cargo Road, Airport Zone, ALPS Ave, Camps, Immigration Checkpoints, Construction Sites, Jurong Island, Jurong Port, PSA Ports, SATS, Aerospaces, Shipyards, Supply Bases

You can provide the four-digit PIN to your family members or a trusted recipient to receive the package on your behalf. In the event that there is no one around to receive the medication parcel, the delivery driver will contact you to re-schedule within the same day.

If the delivery is unsuccessful the second time round, the medication parcel will be returned to the pharmacy. Do note that a re-delivery charge of $8 will be imposed to arrange a new delivery.
